Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. The task which the writer of these lines assumed four years ago is at last concluded. If it has been but imperfectly done, the half loaf is better than no bread. It will be long before the complete and adequate history is written.
